Welcome to Harmony Women's Healthcare. Most women today realize the importance of ongoing health maintenance, regardless of age. Likewise, more than ever, they know they have choices in their healthcare. The healthcare providers at Harmony Women's Healthcare know this also. We hope that you find our staff to be both friendly and efficient. Dr. Broderick-Thomas, Dr. Jackson, and Dr. Subong-Chambers are accepting new patients.

To help facilitate your first appointment, you may want to complete the Gynecology Health History Form prior to coming in for your first visit; only complete the left side of this form. For your convenience, you can also access the Financial Policy Form, and Appointment Worksheet via this website prior to your visit with us. Please print out these forms and complete them. Bring the completed paperwork with you when you come for your appointment.

In addition, it is helpful to obtain medical records from your previous physician(s). To assist our office in obtaining your prior medical records, please print out the Medical Records Authorization for Release of Medical Information Form, complete it, sign it, then fax or mail a copy to your previous physicians' offices. The staff at those offices should then mail a copy of your medical records to our office.
